PDF form issues

We’re aware of an issue which means our PDF forms aren’t displaying properly when being viewed on particular web browsers. 

To ensure that the forms work correctly (either if you’re completing them digitally or printing them out to complete) please first download the form to your computer and open it as a PDF on Adobe Reader (this is free to download if you don’t already have it). This will ensure that all the interactive elements appear for you to complete.
