New Ill-health retirement forms are now available on our website. The key changes are that:-

• they emphasise that if a member needs assistance in completing the forms or collating the information required, they should seek assistance from a third party
• they contain a section with a separate declaration in respect of those persons who have Court of Protection or Power of Attorney, in order to sign on the member’s behalf. (A copy of the Court of Protection or Power of Attorney will also need to be supplied.)
