To assist our Bereavement team in processing Death Benefits cases, we’d appreciate it if all applicable sections of the form 22A could be completed before returned to us. In particular, we’ve seen a few instances where Part A, question 6 “Was Ill-health the main factor in the teacher leaving pensionable employment?”  has been left blank.

There’ve been a number of instances where incomplete information has resulted in delays.
