Use of most up to date version of forms

When submitting (or asking an employee to submit) a paper application form, please ensure that it’s the most up to date version.

We’ve had a number of instances recently where historic versions of application forms have been submitted and subsequently rejected, causing distress to the affected members and delays to the process.

All of the forms available on paper can be found in their most up to date format on our website. If you or a member requires a paper version of a form that’s ordinarily completed as an online webform, please call our Contact Centre on 0345 6066166 (8:30am-6:00pm Monday to Friday).
