Changes to forms

In preparation for MCR we’ve updated several forms, a mixture of both web forms and paper forms. These are:

Webforms

  • Faster accrual – provide role identifier (field will only be visible to those on MCR)
  • Opt Out – provide role identifier (field will only be visible to those on MCR)
  • Certificate of Re-employment - Service tables updated to provide new fields for role identifier and additional pension payments. If the member has previously retired in the tax year selected, you’ll be asked for the date they recommenced employment.
  • Retirement - Service tables updated to provide new fields for role identifier and additional pension payments
  • Transfer in - Service tables updated to provide new fields for role identifier and additional pension payments
  • Repayment of contributions- - Service tables updated to provide new fields for role identifier and additional pension payments
  • Only MCR employers will be required to complete the new fields, they’ll become optional for all other employers.

Paper forms

  • Career average flexibilities - Service tables updated to provide new fields for role identifier and additional pension payments. Member also asked to provide contract name)
  • Opt out form - Service tables updated to provide new fields for role identifier and additional pension payments
  • Opt in form - Service tables updated to provide new fields for role identifier and additional pension payments
  • All service tables within the paper forms will be updated to a new format including role identifier and additional pension payments
  • All employers will see the additional fields

These have been updated to allow for additional information, which will need to be provided by you once you’ve on-boarded to MCR.

Please note, these will only be applicable if you’ve on-boarded to MCR, so you’ll not need to provide this information before then.
